Day Kimball Hospital’s Case Management Department is responsible for the post-hospitalization needs of our patients. This may include skilled nursing, in-home service, or community-based services to assist our patients in regaining their independence after being discharged from the hospital. Case Management also supports patient care through utilization review and comprehensive case management services.
Comprehensive case management and medical social work: Congestive Heart Failure program, Total Joint Replacement program, Chronic Obstructive Pulmonary Disease program; referrals for community services, skilled nursing and rehabilitation facilities; in hospital and after hospital care authorization.
