Our team receives extensive training in the special needs experienced by patients, families and caregivers of those experiencing a serious and life-limiting illness. We understand the need to achieve comfort through pain and symptom control and support needed by those caring for their loved ones.
Care is provided in the care of the patient’s home or other facilities such as area nursing homes if the home is not a possible option.
Our team will help meet your goals, provide information of what you may expect and be available to answer questions and concerns holistically addressing physical, emotional, social and spiritual needs and supports during the illness. Bereavement support is available for family members and loved ones for at least 12 months.
Hospice Services:
Hospice is provided in the home setting and other alternative settings when necessary.

- Through the hospice benefit of most insurance, medical equipment and medications are provided by hospice.
Palliative Care Program
Through the Provides skilled care to persons with a serious or life limiting illness who may be continuing to aggressively treat their illness or who have a prognosis that is greater then six months. Through the palliative care program physical, psychosocial and spiritual needs are addressed through our interdisciplinary team. Our program works closely with Day Kimball Oncology Department, physicians as well as other institutions to provide comprehensive care through close collaboration.
How do I get referred to the program?
Anyone involved in the patient’s care can request information or make a referral. Referrals come from physicians, nurses, discharging healthcare facilities, patients, families, friends and clergy. We encourage anyone involved in the patient’s care to call to request information or make a referral. We are always available to discuss individualized services that will help you
Hours of operation
The hospice office is open weekdays from 8:00 am – 4:30 pm. There is a nurse available 24 hours a day, seven days a week to address concerns by phone or to make a visit when needed.
