Medical Records

Day Kimball Hospital’s Medical Records Department is the administrative arm of the hospital responsible for ensuring that patients’ medical records are complete, accurate, confidential and readily available for future patient care. 

In addition, all birth certificate information is generated from this department and transmitted to the Connecticut Department of Public Health Vital Records Section. 
 


Copies of Medical Records
These can be obtained by the patient upon request. Patients must provide picture identification (driver’s license preferred) for proof of identification.  Please click here to obtain an authorization to release medical records form. Please be sure to fill out this form in its entirety. 

Newborn Social Security Number
New parents can request that a social security number be assigned to their newborn at birth. The Medical Record staff transmits the request to the Social Security Administration. The Social Security number is then sent to the parents within six to eight weeks. Follow up should be with the Social Security Administration.

Paternity Papers
Please call for an appointment to complete paternity acknowledgements for newborns within 8 days of birth. Parents must show picture ID for identification. If the acknowledgement is not signed within 8 days of birth, it must be completed with the Department of Social Services. Please call the Department of Social Services in Norwich, CT at 860-823-3325 for more information.

Birth Certificates
Certified Birth Certificates can be obtained through the Town Clerk of the town of birth or through the State Department of Public Health Vital Records Section. The Medical Record Department cannot issue a certified Birth Certificate.

Copies of Patient Records
These can be released only with the permission of the patient. Only the patient can authorize release of information, unless the patient is under 18 years of age. If the patient is currently less than 18 years old, his parent or guardian must sign the authorization to release the medical records. (For Psychiatric records some restrictions may apply. Please call our office before completing the request and authorization forms.) 

Please bring photo identification, such as a driver’s license, to obtain copies of medical records.
