Cardiac Rehabilitation

Day Kimball Hospital’s Cardiac Rehabilitation is an effective outpatient program to assist patients who have recently suffered a “cardiac event” to live full, quality lives through education and supervised exercise. A “cardiac event” describes several types of serious cardiac conditions, including heart attack, angina, heart bypass surgery or angioplasty. The program also offers at-risk individuals a series of informational classes to build awareness and understanding of heart disease, and to help prevent future cardiac events through lifestyle modification.

Patients participating in the Cardiac Rehabilitation program are referred by their physician or cardiologist. The program is expertly staffed by a team of registered nurses and exercise physiologists who are educated and experienced in this specialty to assist in the recovery from and the prevention of heart disease.

The program is located in two, state-of-the-art facilities at the hospital and at the Plainfield Health Center in Plainfield, CT. Day Kimball Hospital’s Cardiac Rehabilitation Program is certified by the American Association of Cardiovascular Pulmonary Rehabilitation, meeting national standards of excellence in cardiac rehabilitation patient care.

Cardiac Rehabilitation offers three different phases

Phase 2 provides exercise sessions three times a week with full monitoring for the patient who has had a recent heart attack, open-heart surgery, angioplasty/stenting, or stable angina. This phase is covered by most insurance plans. Education classes are held every Tuesday morning from 8:30 am – 9:30am. The education classes are open to the public at no cost. Phase 2 Cardiac Rehab generally lasts up to three months ( 36 sessions ).

Phase 3 also provides exercise and education for the patient who has not had a recent cardiac event but wishes to reduce his/her risk of developing heart disease. Phase 3 is not covered by most insurance plans. This is a self- pay phase, costing $91.30 each month for 2 months. The hospital will bill the patient.

Phase 4 is also known as the “Maintenance Class.” Phase 4 is not covered by most insurance plans. This is a self-pay phase costing $41.50 per month. The hospital will bill the patient. Phase 4 is for people who have graduated from either the Phase 2 or 3 programs and wish to continue with their exercise under the supervision of the cardiac rehabilitation staff. Individuals who have graduated from other cardiac rehabilitation programs are also welcome.

Plainfield Health Center:
Only Phase 4 (Maintenance) is provided at this site. This is a self-pay phase costing $41.50 per month. The hospital will bill the patient. Phase 4 is for people who have graduated from either the Phase 2 or 3 programs and wish to continue with their exercise under the supervision of the Cardiac Rehabilitation staff. Individuals who have graduated from other cardiac rehabilitation programs are also welcome.

What to expect during your visit/procedure

Your first visit to Cardiac Rehabilitation consists of an interview and thorough assessment of your medical history. You will be asked to fill out informational forms, which help us to plan for your education needs and your exercise prescription. This visit will take about an hour. There is no exercise session during the first visit.

Important reminders/things to remember

- For the first visit, please bring in your insurance card(s) and a list of your medications (prescription and over the counter.)

- For exercise classes bring in a clean pair of sneakers (or a similar type of shoe). Wear loose, comfortable clothing.

- If you need to cancel your visit or your exercise class, please call as soon as possible and we will reschedule your appointment.
