Case Management Department
Day Kimball Hospital’s Case Management Department is responsible
for the post-hospitalization needs of our patients. This may include
skilled
nursing, in-home service,
or community-based services to assist our patients in regaining their
independence after being discharged from the hospital. Case Management
also supports patient care through utilization review and comprehensive
case management services.
Services Provided
Comprehensive case management and medical social work: Congestive Heart
Faliure program, Total Joint Replacement program; referrals for community
services, skilled nursing and rehabilitation facilities; in hospital and
after hospital care authorization. Also provides "Healthy Start/HUSKY" services,
a state supported insurance program for pregnant women, infants and children,
and compresensive case management for pregnant women at high risk.
What to expect during your visit/procedure
If you need the services of the Case Management Department, a case manager
will visit with you while you are in the hospital.

Healthy Start and “HUSKY” Program
These are no cost to low cost health care programs for pregnant women and children up to 19 years of age. Services include prenatal care for pregnant women and complete medical coverage for children. Parents or guardians may qualify for free medical assistance if financial guidelines
are met.
