Assessment Program
Day Kimball Hospital’s Center for Healthy Aging offers a variety
of health, educational and other services for older adults age 60 and older
living in northeastern Connecticut, with the goal of helping them maintain
optimal health and independence.
The diversified wellness services offered by the Center for Healthy Aging
include: health screenings, such as blood pressure, footcare, bone density,
and breast exams; geriatric evaluations including functional assessments
and evaluation of memory loss; caregiver consultations; as well as legal
and financial counseling.
The Center’s staff includes a physician and an advance practice registered
nurse who specialize in geriatrics, a registered nurse, a cosmetician specializing
in non-diabetic foot care and an elderlaw attorney. The Center for Healthy
Aging works closely with Day Kimball Hospital staff, local physicians and
community agencies to coordinate care services.

Who is eligible for services?
Older adults who have an existing primary care doctor are eligible. (Referral from your doctor is suggested.)
How do I get an appointment?
Appointments can be made by calling 860-779-9270. A full written
report will be sent to your primary care doctor after your visit.
