Diabetes Self-Management Education ProgramDiabetes is a chronic, progressive illness that affects over 16 million Americans in the United States. While people living with diabetes can live long, active lives, the disease requires the proactive participation of the patient on a day-to-day basis to ensure optimum health, to monitor symptoms, and to stay alert for possible complications so often associated with diabetes. With this in mind, Day Kimball Hospital developed an outpatient Diabetes Self-Management Education Program. This program, recognized by the American Diabetes Association, offers a series of informative, hands-on educational sessions to both adolescents and adults living with Type 1, Type 2, pre-diabetes and gestational diabetes. Participants in the program learn to understand diabetes, including the importance of daily blood glucose testing, how nutrition and exercise positively impact diabetes, and many other important topics related to diabetes. The multi-disciplinary staff of the Diabetes Self-Management Education Program consists of an advanced practice registered nurse /certified diabetes educator, a registered dietitian/certified diabetes educator, an advanced practice registered nurse in behavioral health and a master degreed exercise physiologist. Referral from a primary care physician is necessary to enter the program Services ProvidedPatients receive one-to-one instruction in a classroom setting. Classroom instruction is provided by certified diabetes educators, dietitian, a behavioral specialist and an exercise physiologist. Individual instruction is done to teach home blood glucose monitoring skills and meal planning. The affiliated “Quiet Corner Diabetes Club” provides monthly community education lectures on diabetes and related topics at Day Kimball Hospital. What to expect during your visit/procedureA plan of care will be developed in collaboration with your primary care physician that will enable you to understand the importance of diabetes self-care through meal planning, exercise, blood glucose testing at home and medication management. Important reminders/things to rememberIt is the patient’s responsibility to check with their insurance carrier whether a referral or prior authorization is necessary prior to scheduled visit. |
