Over the past two decades, Day Kimball Hospital’s Pediatric Centers have earned a reputation in the community for providing outstanding personalized health care for children, from newborns to adolescents.
The three centers, conveniently located at the hospital in Putnam, in the Plainfield Health Center, and in the Thompson Health Center are expertly staffed by a team of board certified pediatricians, pediatric nurse practitioners and nurses. These providers, many of whom
have special interests in areas such as asthma and adolescent health, emphasize the importance of getting to know “their families,” and strive to include parents and guardians in the treatment of
their children.
The Pediatric Centers use a “primary care practice” model. Parents
or guardians and their children can select a pediatrician or nurse practitioner
as their care provider. Healthcare services provided include sick visit
care, immunizations, check ups, as well as school and camp physicals.
Highly specialized services offered include
the evaluation and treatment
of attention deficit disorders and the evaluation of behavioral health
problems through the services of a board certified child psychiatrist.
To make it easier for working parents, the Putnam Pediatric Center offers evening appointments Monday through Thursday.
Services Provided
Sick visit care, immunizations, check ups, as well as school
and camp physicals. Specialized services include evaluation
and treatment for attention deficit disorders.

What to expect during your visit/procedure
There is usually a minimum wait to see the doctor or nurse practitioner. If a wait is required, it is because we are busy with
other patients in need. We ask for your understanding and patience.
Important reminders/things to remember
• Appointments are required
• Appointment cancellations should be made no later than 24 hour prior to scheduled visit
• Please bring all medications that your child is currently taking at
the time of your visit
• Insurance cards and copays must be presented for each visit
The Day Kimball Hospital Immunization Action Plan
The Day Kimball Hospital Immunization Action Plan (IAP) works with the
State of Connecticut Immunization Program. It provides referral services
for physicians to help them find children who may be behind on immunizations
(shots). It also works with parents to help them find pediatricians to get
all of the shots their children need.
The IAP program also works with CIRTS, the Connecticut Immunization Registry
and Tracking System, which is a computerized shots registry. It keeps shot
records for preschool-aged children, even if they move from Connecticut,
their pediatrician retires, or they change doctors and forget their shot
history. These records are necessary for when a child enters daycare, school,
camp or even college.

Putnam Pediatric Center
The Pediatric Center is in the Medical Office Building, located in back of the hospital. Drive in to the hospital’s main entry road and continue straight towards the Emergency Department. Bear to the right in front of helicopter landing pad and drive towards the first row of parking. Take a right at the first row. The Medical Office Building is a brick building on your right hand side. Parking is
in front of the building.

Well Child Physicians Service Program
This program offers financial assistance for well child medical visits for children 5 and under. Income restrictions apply. Contact Janet Johnson, (860) 963-5331.
Women, Infants and Children
(WIC) Program
This program offers nutrition education as well as formula, eggs, milk, juice, cereals, cheese, dried beans and peanut butter. It is an income eligible program. Contact Wendy Osborn, (860) 928-3660.
Healthy Start and “Husky” Program
These are free health care programs for pregnant women and children up to 19 years of age. Services include prenatal care for pregnant women and complete medical coverage for children. Parents or guardians may qualify for free medical assistance if financial guidelines are met. For information call 963-6415. |
