Day Kimball Healthcare is pleased to partner with the town of Plainfield and its school system leadership to hold a community COVID-19 vaccine clinic at the Plainfield High School, 105 Putnam Road, Central Village, CT. The clinic will be administering the Moderna vaccine, which includes two doses administered 28 days apart.
The state of Connecticut's age eligibility requirement is currently open to those age 45 and older. Anyone who lives or works in Connecticut within the eligibility requirements can sign up to receive a vaccine at the clinic. Find out if you're eligible by clicking here >
Individuals can register for the clinic in VAMS. Click here to register.
Remember to bring a form of ID and insurance card. Please wear a mask and clothing that will provide easy access to upper arm.
