Changes include expanded lab hours, consolidated clinic locations, and enhanced patient convenience

As part of Day Kimball Health’s (DKH) ongoing commitment to enhance patient access and improve operational efficiency, the outpatient services currently located at 12 South Main Street in Putnam will be relocated to more centralized and convenient locations. 

Effective Sept. 2, 2025, laboratory and blood draw services at 12 South Main St., Putnam, will be consolidated with the hospital’s main laboratory at Day Kimball Hospital, 320 Pomfret St., Putnam. To ease the transition for patients, hospital lab hours will expand Monday through Friday from 6 a.m. to 8 p.m. Additional parking spaces will be reserved for lab patients, with signage marking the spaces and displaying a dedicated phone number for those who need assistance entering the building.

Lab and blood draw services will also continue at:

• 55 Green Hollow Road, Danielson, Conn.
• 12 Lathrop Road, Plainfield, Conn.

On the same date, the Anticoagulation Clinics currently at 12 South Main St. in Putnam and 12 Lathrop Road in Plainfield will consolidate into a single location at 55 Green Hollow Road in Danielson.

The relocation of Physical Medicine and Rehabilitation services from 12 South Main St., Putnam, to 35 Kennedy Drive in Putnam, is targeted for Oct. 1, 2025. At the new site, services will be co-located with The Ortho Group of DKH. Services will also expand at 55 Green Hollow Road in Danielson, bringing related care together under one roof to improve coordination and convenience for patients.
