You face some kind of health care issue nearly every day. The critical first step towards addressing your concerns and improving your family’s health is establishing a relationship with a primary care practitioner (PCP) for everyone in your family. 

At Day Kimball Healthcare, your PCP is at the core of our patient-centered medical home (PCMH) approach to healthcare. In this model, your PCP doesn’t just care for your day-to-day health needs and treat you when you’re sick; he or she also serves as your healthcare advocate by coordinating all aspects of your care throughout our system.

Our primary care practitioners:

  • Work to keep you well by providing preventive care and by helping you to proactively manage your own health, thus improving your quality of life and reducing your healthcare costs
  • Are there for you with same-day sick appointments when minor illnesses or injuries do arise, offering both savings and better continuity of care over a trip to a walk-in center or emergency department. And when your primary care office is closed, our Walk-In Center provides evening and weekend urgent care that is still coordinated with your PCP.
  • Provide not only referrals but comprehensive care coordination across all the services Day Kimball Healthcare has to offer - so when specialized care is required you can be assured that all of your providers are working together within an informed and holistic approach that is tailored to you.

Family Medicine
Family medicine practitioners provide continuing, comprehensive healthcare for individuals of all ages (from infancy to elderly) and both genders, as well as care for each organ system and every disease entity. As the name implies, family medicine physicians can also provide care to all members of a family, offering continuity of care within the household. Internal Medicine
Internal medicine practitioners provide adults 18 years of age or older (or as young as 16 in some practices) with comprehensive care, including sick and well care visits, preventive care, women’s health services, minor office procedures, and work-related physicals. Learn more…

Pediatricians provide comprehensive care for children from birth through adolescence and into young adulthood. Services include sick and well care visits, physicals, immunizations, school and camp physicals, and specialized services including the evaluation and treatment of asthma, ADD/ADHD and adolescent medicine. Walk-In Care
When you can’t get to your primary care practitioner, we’re still here for you. Our Walk-In Center offers evening and weekend hours so you have somewhere to turn to for urgent care when your PCP’s office is closed. This is a more efficient and cost-effective choice than the emergency department for minor illnesses and injuries. Enhanced coordinated care programs

These programs, available only through our PCPs, offer enhanced coordinated care for populations with complex and ongoing healthcare needs.

Diabetes Management
This program helps adolescents and adults with Type 1 or Type 2 diabetes, pre-diabetes, or gestational diabetes to manage their condition and maintain their health. A specialized care plan will be developed in collaboration with your primary care physician that will enable you to understand and perform diabetes self-care through meal planning, exercise, at-home blood glucose testing and medication management. Geriatric Care Management
This area of care offers comprehensive evaluation, planning and care coordination for adults age 60 or older. It helps patients and their caregivers manage chronic illnesses and high-risk conditions to improve health, safety and functionality. Learn more…
