Family medicine practitioners provide continuing, comprehensive health care for individuals of all ages. As the name implies, family medicine practitioners can provide care to all members of a family, offering continuity of care within the household. Some family medicine physicians also provide obstetrical care for women.
The physicians and Advanced Practice Registered Nurses (APRNs) in our family medicine practices focus on helping their patients achieve optimal physical and mental health through accessible, safe, cost-effective care that is based on best evidence, responsive to the needs and preferences of patients, and respectful of the patient’s family, personal values and beliefs.
Convenient Office Hours and Sick Visits
When a minor illness or injury arises, your primary care team is there for you with acute care appointments. This provides not only convenience but also continuity of care, allowing you to be seen and treated by a practitioner right within the same medical group where you already have an established history of care. Because it is billed as an office visit, it also helps you to avoid the higher cost of urgent or emergency care.
Our Integrated Network of Care
As your “Patient Centered Medical Home” (PCMH), your family medicine physician and the rest of your primary care team also coordinate and facilitate your care throughout our system. To aid in this effort, we’ve designed our integrated system to allow your primary care team and our walk-in center, emergency department, specialists, diagnostic imaging technicians, laboratory technicians and other healthcare professionals to “talk” about your health through electronic medical records that provide instant access to your medical information. This allows your entire care team to make an informed and comprehensive evaluation of your condition and to provide top-quality care within a truly integrated and holistic approach that also eliminates errors and redundant services to help you control your healthcare costs.
Patient-Centered Medical Home (PCMH)
Our Primary Care practice locations are your homes for patient centered care. We’re in pursuit of developing a patient-centered medical home (PCMH) approach to health care. In this model, the PCP serves as your health care advocate, coordinating all aspects of your care throughout our system.
Patient-centered medical homes (PCMH) are transforming how primary care is organized and delivered. The Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care. The medical home encompasses five functions and attributes:
- Comprehensive Care: Provided by our Day Kimball Medical Group team of care providers, including physicians, physician assistants, nurses, care coordinators, and others.
- Patient-Centered: The primary care medical home provides health care that is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences.
- Coordinated Care: The primary care medical home coordinates care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports. Such coordination is particularly critical during transitions between sites of care, such as when patients are being discharged from the hospital.
- Accessible Services: The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team, and alternative methods of communication such as email and telephone care.
- Quality & Safety: The primary care medical home demonstrates a commitment to quality and quality improvement by ongoing engagement in activities such as using evidencebased medicine and clinical decision-support tools to guide shared decision making with patients and families, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction, and practicing population health management.

Billing for primary care office visits will come from Day Kimball Medical Group. Please check with your insurance carrier about your specific coverage, copays and deductibles as well as any prior authorization requirements.
